NDIS Report Guide

  • Summary of therapy progress to justify continued support. Best for plan extensions.

    Requirements:

    • 3+ sessions with clinician

    • Summary of therapy outcomes

    • Data on goal progress

    • Recommendations for future goals

    Funding: 2-3 hours

  • Progress report requesting variations to the NDIS plan e.g. budget changes, low-cost AT, specialised assessments (like an FCA).

    Requirements:

    • 3+ sessions with clinician

    • Summary of therapy outcomes

    • Clinical evidence justifying changes

    • Assessment supporting the variation

    Funding: 3-4 hours

  • Submitted when functional capacity or environment changes significantly mid-plan.

    Requirements:

    Evidence of the "Change of Circumstance". 

    For example:

    • Health Deterioration

    • Diagnosis change 

    • Loss of informal supports

    • Change in living arrangements 

    • New goals or life stage transition e.g. leaving school

    • Unexpected and significant event

    Funding: 2-4 hours

  • Quick summary of progress or recommendations, often used if funding is too limited for a full report.

    Requirements:

    • 1-2 sessions with clinician

    • Summary of need & link to NDIS goals

    Funding: 1-2 hours

  • Clinical recommendations for safe swallowing and mealtime enjoyment, to prevent choking or aspiration. 

    Requirements:

    • At least 1 mealtime review session with the Speech Pathologist

    • Additional review sessions for more complex presentations or varying presentation.

    Funding: 1-2 hours

  • Review of all domains of function, most often for:

    • NDIS Access

    • Change of Circumstance review

    • Adding new diagnosis

    Requirements:

    • FCA not completed within 1 year

    • Evidence and clinical assessment to justify changes (e.g., WHODAS, Vineland)

    • 3 + sessions with OT.

    Funding: 10-15 hours

    Depending on complexity and detail required.

  • Determines high-level housing support and staffing ratios. To support access to SIL, ILO and SDA. This will include:

    • Functional Capacity Assessment

    • Staffing ratios

    • Housing requirements

    Requirements:

    • Functional Capacity Assessment

    • Staffing ratio evidence

    • Housing type evidence

    • Other options explored and not appropriate or available

    • 3 + sessions with OT.

    Funding:

    SIL/ILO 15-20 hrs

    SIL/SDA 20 hrs+

  • Requesting funding from the NDIS for Assistive Technology that costs $1,500 – $15,000. For example:

    • Electric beds and mattresses

    • Manual wheelchairs

    • Commode

    • Hoist & Sling

    • Communication Device

    Requirements:

    • Trial of at least two items

    • Outcome measures

    • Complete NDIS General Assistive Technology

    • Assessment template

    • AT quotes

    Funding: Approximately 10 hours per item

  • Requesting funding from the NDIS for Assistive Technology that costs >$15,000. For example:

    • Power Wheelchair

    • Communication Device with alternative access e.g. eye gaze

    Requirements:

    • Trial of at least two items

    • Trial at 2 different suppliers (if able)

    • Outcome measures

    • Complete NDIS General Assistive Technology

    • AT quotes

    Funding: Approximately 20 hours per item

  • Requested when current AT or equipment is broken, outgrown, or no longer fit for purpose.

    Since 03/10/2025, NDIS requires this for items on the ‘not funded’ list. These include:

    • Smart watches

    • Tablets/ iPads

    • Communication apps

    Requirements:

    • Evidence that current AT is no longer fit for purpose.

    • Quote for replacement.

    • Evidence of AT trial.

    Funding: 2-4 hour
